Employee

General Requirements

  • Original Passport
  • 2 pcs passport size photos
  • Photocopy of passport biopage and visa stamps
  • Application Form
  • Original Bank Certificate (with ADB, current balance and Embassy address where you will applying)
  • Bank Statement (Latest 6 months)
  • Original PSA Birth Certificate
  • Original PSA Marriage Certificate (if needed)

Employment Documents

  • Certificate of Employment
  • Photocopy of Company ID (front and back)
  • Company Payslip (latest 3 months)
  • Leave of Absence
  • ITR 2316

Business Owner

General Requirements

  • Original Passport
  • 2 pcs passport size photos
  • Photocopy of passport biopage and visa stamps
  • Application Form
  • Original Bank Certificate (with ADB, current balance and Embassy address where you will applying)
  • Bank Statement (Latest 6 months)
  • Original PSA Birth Certificate
  • Original PSA Marriage Certificate (if needed)

If Single Proprietorship

  • DTI Certificate
  • Mayor’s Permit
  • BIR 2303
  • ITR 1701

If Corporation

  • SEC Certificate
  • Mayor’s Permit
  • BIR 2303
  • ITR 1701

Retiree

General Requirements

  • Original Passport
  • 2 pcs passport size photos
  • Photocopy of passport biopage and visa stamps
  • Application Form
  • Original Bank Certificate (with ADB, current balance and Embassy address where you will applying)
  • Bank Statement (Latest 6 months)
  • Original PSA Birth Certificate
  • Original PSA Marriage Certificate (if needed)

Additional Documents

  • Photocopy of Senior ID (Front and back)
  • Proof of retirement (optional)

If Sponsored

  • Original Bank Certificate (with ADB, current balance and Embassy address where you will applying)
  • Bank Statement (Latest 6 months)
  • Employment or Business documents of Sponsor

Student

General Requirements

  • Original Passport
  • 2 pcs passport size photos
  • Photocopy of passport biopage and visa stamps

  • Notarized Affidavit of Support

Student Documents

  • Photocopy of School ID (Front and back)
  • Certificate of Enrollment
